When you click on the link for a Downloadable Form, the item will open in Adobe Acrobat or Acrobat Reader. If you do not have Acrobat Reader, please click

You may either fill out the form on your computer, or print out the form and fill it in manually.
If you fill out the form on your computer, you will then have two options:
- You can save the document to your computer for later printing. Then either mail the completed form to Mill Springs Academy or bring it with you to the school when you visit.
- You can click on the "Submit Form" button at the top right of the screen. This will send the filled-out form to Mill Springs via whichever email client you choose. You will be contacted for an appointment. It is recommended you save a copy for your own records.
